What the letter says

Hoag’s patient letter, dated September 28, 2026

  • The contract between Hoag and the plan for Medicare Advantage HMO members expires on December 31, 2026.
  • During October and November, the plan will assign affected members a new primary care physician who is not part of Hoag.
  • Beginning January 1, 2027, those members will no longer have access to Hoag specialists or elective inpatient and outpatient services at Hoag facilities. These include Hoag Hospital Newport Beach, Hoag Hospital Irvine, Hoag Orthopedic Institute, Hoag Medical Group, Hoag Specialty Clinic, and others.
  • Emergency care at the Hoag hospitals is not affected.
  • Members may have “continuity of care” rights for a limited period, as decided by the plan.

Your options before December 7

Three ways to handle it

1. Stay in your plan

You keep your plan, but beginning January 1, 2027 you lose your current Hoag doctors and access to Hoag facilities, except for emergency care. The plan assigns you a new primary care physician who is not part of Hoag.

2. Move to another Medicare Advantage plan

There are still Medicare Advantage plans that work with Hoag for 2027. If you make the change before December 7, you can keep your Hoag doctors and hospitals. I check each of your doctors and the Hoag facilities you use against the plan before you enroll.

3. Original Medicare with Medigap and Part D

Hoag accepts Original Medicare with a Medigap (Supplement) plan. You may qualify for a Medigap plan without medical underwriting, depending on your situation, and I can check that for you.

My job is to show you the options and the costs, not to tell you which one to pick. There is no cost to you for a review.

Not sure which option fits you?

Call me and bring your Hoag letter. I will check your doctors, your hospital and your medications, and show you what each option would look like.

Key dates

Why December 7 matters

  • October 15 – December 7: Annual Enrollment. You can change Medicare Advantage or Part D plans, and the change starts January 1.
  • January 1 – March 31: the Medicare Advantage Open Enrollment Period lets people already in a Medicare Advantage plan make one change. If you wait, you would be without Hoag specialists from January 1 until the change takes effect.
Common questions

Questions from Hoag patients

Does this affect my emergency care at Hoag?

No. Hoag's letter says emergency care is always considered in-network and is not affected by this change. If you or a family member has a medical emergency, go to the nearest emergency department.

Can I keep seeing my Hoag doctor for a while?

You may have continuity of care rights for a limited period if you are in the middle of treatment with a Hoag provider. Whether you qualify is decided by your plan, and the time is limited. Call me and I will walk you through what your letter means for you.

How do I know if this applies to me?

Read the letter you got from Hoag. It says which plan the change applies to. If you did not get a letter or you are not sure, call me and we can go through your coverage together.

Will switching plans cost me more?

It depends on the plan. Premiums, copays, and your prescriptions all differ. Medigap premiums are usually higher than a Medicare Advantage plan, but you can see any provider that accepts Medicare, including Hoag. I walk through your doctors, your drugs, and the total cost under each option I review so you can decide.

Want to see which doctors are in which plans?

Call, book online, or ask for a call back. I will go through your Hoag letter with you and your doctors and drugs.
